We welcome any person wishing to register who lives within the postcode of areas of G3 (not beyond the Kelvinhall), G11 and G12 and who is entitled to NHS treatment. Please attend the practice only if you cannot download the application form and the receptionist will give you a registration form to take away for your to complete and return to us.
Each person registering is asked to complete a registration form (known as a GPR), and a Health Questionnaire.
Please download all three forms below, and if possible but not mandatory, one form of ID and email to the practice website.
On receipt of the email we will be in touch to inform you if your registration has been accepted or not, and if not why not e.g our practice list is closed, the patient lives outwith the practice area etc.
You should allow 7 days for this process to be complete.

Registration Policy
In accordance with Health Board name recommendations, no documents are required to register with a GP although staff may ask you to provide documents of identification if the Health Board cannot trace any previous registrations or is a first registration with the Practice.
One form of identity must ideally be photographic such as:
- Passport
- Drivers Licence
- Official ID card from Public Services body
- Student matriculation card (current year)
If unable to provide photographic evidence then you must present one of the above and one of the following:-
- Birth Certificate
- Marriage Certificate
- DivorceAnnulment Papers.
N.B You will still be able to register if you are unable to provide any of these documents.
