It is very important that you notify the Practice if you have moved. We require your current address details, so that we may contact you regarding your personal health.
If you do not inform the Practice of your correct address you may be removed from the list. Our local Health Board routinely asks GP Practices to check patient addresses to establish if they are still currently registered at the address provided by the patient. If your GP Practice is unable to confirm your details, then the Health Board may remove you from the Practice List.
You can let us know of your change by telephone, in person at the surgery, or online. We will then update your medical records accordingly. If your new address is out-with our Practice core area code then you will be informed to register with a GP in your local area.
Your personal information provided, will be solely used to process your change of address request. It will not be passed to any third party, without your consent, unless required by law. This conforms to the UK Data Protection Act of 2018 (GDPR).
